- Valid Photo ID
The signer must show an unexpired, government-issued photo ID (driver’s license, state ID, passport, etc.).

- – Do not sign the document in advance
- – If multiple signers are involved, all must be present with ID
- Witnesses (If Required)
Some documents—like powers of attorney, living wills, or real estate deeds—may require two adult witnesses.
- – Witnesses cannot be named in the document or related to the signer
- – Most facilities do not allow staff to act as witnesses
